Application Process

• We accept children ages 2-12.
• Children do not need to be toilet trained.
• Our center is open to the public, however, UND Apartment residents receive a discounted rate.
• You may request an information packet online or by calling us at (701) 777-3947.

STEP ONE: COMPLETE APPLICATION AND OTHER FORMS LISTED BELOW

3. Child Information Sheet
4. Class Schedule
5. Educational Data for Child's Teacher
6. Parent's Statement on Health of Child
7. USDA Form
8. Immunization Form

STEP TWO: ATTACH APPLICATION FEE
Attach required $30.00 non-refundable application fee to completed application. Please make check payable to UND.

STEP THREE: RETURN APPLICATION
Return your complete application and application fee of 30.00 to:

You will want to return your application promptly as new applicants will be served in the order applications are received. Parents will be notified if there is NO space available for their child. If there is no space available, your application and application fee will be returned to you.

STEP FOUR: ATTEND PARENT ORIENTATION
A Parent Manual and UCC Calendar will be given to you so you may become familiar with UCC's policies. Parent Orientation will give us the opportunity to discuss UCC's program and policies and address any questions or concerns you may have. If UCC is new to you as a parent, please make arrangements to attend the Parent Orientation Sessions. You will be notified when these sessions will be held.
